Eligibility

By booking or using our services, you confirm that:

Appointments, Cancellations, and Payments

1. Appointments must be scheduled in advance

2. Cancellations require at least a 24 hr notice in advance

3. Payment is due at the time of service unless otherwise agreed in writing

4. We accept Credit and debit card payments

Insurance

1. We are out-of-network with insurance providers, therefore clients are responsible for payments and may submit for reimbursement directly from the insurer

Limitation of Liability

1. While we strive to provide high-quality care, by using our services you acknowledge and agree that:

2. You participate in physical therapy services at your own risk and agree to follow the instructions of your physical therapist.
